Worked my way down the truobleshooting list.
  Got it to work on step 16.
  "If you want to enable S/PDIF audio output, then unmute and increase 
volume on the S/PDIF and S/PDIF playback channels in the alsamixer 
application. Use the M key to toggle between muted (MM) and unmuted (00) 
states. "
  My alsamixer shows the following:
  1. s/pdif
  2. s/pdif d
  3. s/pdif p
  4. s/pdif 1

I think i have installed something twice. duh!
  So what worked was unmuting s/pdif 1. This one does not have any 
volume bars on top of it, just a MM which turns into a 00.
