[ubuntu-za] Failed RAID boot

Hi Guys

I just got some brand new Intel Blade servers to install Ubuntu server
on.  But for the first time I'm not able to get it to run.

In short I have 2 500GB hard drives with 3 partitions each combined into
3 RAID1 partitions as follows:

/dev/sda1 + /dev/sdb1 => /dev/md0 => /     (45 GB)
/dev/sda2 + /dev/sdb2 => /dev/md1 => /kvm  (450 GB)
/dev/sda3 + /dev/sdb3 => /dev/md2 => swap  (5.1 GB)

The installation process completes without a glitch, but the boot fails
with the following output:

  mount: mounting /dev/disk/by-uuid/a3c31122-6794-4fa6-a039-91d49dadfa8a
  on /root
  failed: Invalid argument
  mount: mounting /sys on /root/sys failed: No such file or directory
  mount: mounting /dev on /root/dev failed: No such file or directory
  mount: mounting /sys on /root/sys failed: No such file or directory
  mount: mounting /proc on /root/proc failed: No such file or directory
  Target filesystem doesn't have /sbin/init.
  No init found. Try passing init = bootarg.

  BusyBox v1.13.3 (Ubuntu 1:1.13.3-1ubuntu7) built-in shell (ash)
  Enter 'help' for a lit of built in commands.

  (initramfs)

I haven't yet found something on google that solves the problem.
Anybody else that got it fixed?  

I have two (brand new) blade servers, and both do the same thing, so it
does not appear to be a hardware failure.

Important Specs:
- SR1600URHSR - INTEL 1U URBANA PLATFORM SR1600URHS (REFRESHED) 
- ASR1500SASBP - SR1500 ACC SAS BACKPLANE 
- 1 x BX80602E5506 - INTEL XEON 5506 - 2.13GHz 
- 3 x TS256MLK72V1U - TRANSCEND 2GB DDR3 1066 ECC DIMM 7-7-7 
- 2 x ST3500418AS - SEAGATE 500GB SATA 7200RPM 16MB CACHE
