Have a look at:
https://bugs.launchpad.net/ubuntu/+source/initramfs-tools/+bug/129910
to see if perhaps that might be your problem. Try taking out any vga=xxx
options. You mentioned that this occured after dpkg-reconfigure debconf
so the above might help.
