LTS Release Schedule?

I may be mistaken but I think the answer to the first part of your 
question is: just before the end of life for the current LTS, so several 
  years away.

The second part I don't know, it's a good question and it'd be amazing 
if the answer was that you could 'hop' from one LTS to the next. But in 
practical terms I don't see how it could be done, the technology would 
have changed so much. It's  very much hit and miss just upgrading after 
six months - although to be fair it has been worked on an awful lot and 
upgrading for one release to the next works better than it used to.
