That "io error" in the end seems a little bit worrying. To me, it
looks like the system can't find or head something in the disk.

If it really was a hardware failure, another message about the device
being "resetted" should've appear. It doesn't seem to be the case.

I would suggest to backup everything and reformat it. But I'm not
really sure if it will work.
