[ubuntu-in] New format for CDRequest page

The CDRequest page
http://www.ubuntu-in.org/wiki/CDRequest

has been changed with a new format making it easier to
make a request based on 1) city, 2) distro and 3)
architecture, as mentioned on the page.

Chennai and Mumbai tables are almost ready, and I
would request the other wiki users to create and
update their tables with the most recent state of
their resources. Then, we can do away with the old
table. 

Also, while entering information please specify which
version of *buntu you have, apart from the type of
media i.e. DVD or CD. 

As far as possible, please provide an email address
for contacting and avoid mobile phone numbers. (IMHO)

Do mention whether you'll like to accept a blank CD-R
/ DVD +/- R disc or whether you would like to get
paid, the cost of media.

Also, for those who haven't, please create your
User:<username> page, while including a link within
the CDRequest page.
