Credits and Contributor Attribution

Yesterday it was agreed at the meeting to standardise our methods of
crediting documentation.

At the moment, some documents have individuals listed as authors (e.g.
[1]) and others have the team as the author (e.g. [2]).

The latter is the proper way of doing this: so from now on all documents
should do this:

 * the <authorgroup> tag (to be found inside the <bookinfo> tag) should
have the team listed.
 * the <legalnotice> tag (also inside <bookinfo>) should have two lists,
one with the current maintainer, and another with the other contributors
to the document.

For a perfect example, see the Packaging Guide[3], which Jordan has
already updated.

Scrollkeeper omf files should list the team as creator, the maintainer
as maintainer, and individuals as contributors. Again, for an example,
see the Packaging Guide[4].

A number of documents need updating for this change.
