Sounds like this should happen in all applications and not only in 
firefox + flash. The setting in gnome-sound-settings is kind of obsolete 
now and always has to be set to pulseaudio server. You have to select 
your headset as the default device in the pulseaudio devicechooser or 
you can move the stream in the pulseaudio volume manager. You can also 
enable simultaneous output on all soundcards in the pulseaudio 
preferences and select that as your default device - maybe that should 
be the default configuration for Ubuntu?
