It's been said many many times by Matt Zimmerman, and he was dead-on:
apt-get dist-upgrade NEVER worked without glitches. It cannot by
principle. You always had to have an eye on what it tries to remove. You
always had to run it 2 or three times. And so on. Every Debian users
should know this. I do, and I have run Debian from slink to sarge.
