Ubuntu 16.04.1, no orca

On some machines/ubuntu installations I've had to press the orca hotkey combo twice to get speech, and on some installations I'[ve had to press f10, right 
arrow to accessibility and up to screen reader and press it that way rather than using the hotkey.
