Ironing out the upgrade experience

Martin, et al,

I've updated http://bazaar-vcs.org/FormatUpgrades to give *very* basic
instructions on how to upgrade to 2.0 format.  I am sure someone who
knows more could improve it greatly.  It's not great, it's just better
than no information at all.

In the meantime, I've linked from that page to this thread and to Ian's
upgrade guide.
