Unfortunately, I believe this is an issue with the site and not Firefox.
It could be your session expired with the survey site and you would need
to start again.  This type of error is usually due to the web server
returning an  empty page for the URL in question.  Please report any
other issues you may find.  If you find that this happens again in a
short period of time (less that 10 minutes), please either change the
status of this bug back to New with the updated information or file a
new bug.  Thanks.
