I went to the phpbb SVN  repository and downloaded the phpbb 3.0.4 database_update.php:
http://code.phpbb.com/repositories/entry/phpbb/tags/release_3_0_4/phpBB/install/database_update.php?format=raw

I then created /usr/share/phpbb3/www/install and placed the file there.
I went to http://<myserver>/phpbb3/install/database_update.php in
Firefox and the database upgrade worked first time.  I had to delete the
install directory I created before I could access the forum, but it is
now back to a functional state.
