[Bug 355654] [NEW] Please merge xmlrpcpp with cvs

Nice to see because the Xmlrpcpp coders did upgrade the source code in
cvs 10 months ago.

Done are clean up's win64, ssl, patches. Hack in support for http11 100
continue headers from server. Timeout for client calls, clarify time
arguments as being in seconds. Updated the documentation.
