I have exactly those symptoms, but only in one document. I assumed the 
document contents had got corrupted and have copied the text (not the 
formatting) to a new document and so far the problem has not recurred.

Are you using any non-standard (user installed) fonts in this document? 
I suspected a problem with font metrics.
