You specify the mount point of the partition that is to be your home
directory as /home in /etc/fstab.
E.g. on my desktop machine I have (among other things):-
UUID=2d6ff70c-1894-4afa-a35c-5abb40549d3b / ext4 errors=remount-ro 0 1
UUID=7e8a314e-7eea-4f43-b8ed-9db025e41cbf /home ext4 relatime 0 2
As you say it can make upgrades easier as you can just keep your /home
unchanged while the OS gets updated.
