Ubuntu freezing frequently

Hi,

My laptop, HP Pavilion dv 6000t, is running on Ubuntu 12.04 OS. For last ~2
weeks it is freezing very frequently. When I start the laptop, initially it
seems behave well, but after sometime, it starts freezing. When It freezes,
if I wait for a couple of minutes, then it again starts working but only to
freeze again. My laptop is 5 years old. About six months ago there was a
hard disk crash and thus I had to replaced that with a new one.

Now how do I ensure whether my laptop is dying or it is due to my operating
system or due to some non-hardware related issues ?
