[1] Try a different card.
[2] Try the suspect card in a different machine and/or different reader.
[3] Try under a different OS, e.g. a LiveCD.
You need to isolate exactly what's going wrong.
I have seen solid-state Flash media just die for no apparent reason,
several times now: CF, SD, microSD.
