When headphones are plugged in, the speaker does not get muted automatically.

Please do. I pulled out the CD's & tried: 8.04, 9.04, 9.10 & 10.10 all
from the liveCD's and get the same problem. So it doesn't appear to be a
user setting issue.

Yeah... had a similar situation, so I dual-booted to Win7 in order to
listen to a presentation via the headphones... forgot that I'd not
configured Win7 yet & the infamous Windows startup sound went off &
scared the crap out of me and all around me. I'd turned off the infamous
Ubuntu drum crap previously & so wasn't expecting the startup sound from
Win7 at the time.
