Sometimes the differences between programs are so great there is no
good "translation" between them. The version of the .doc file OOo
produces may not support background images. I cannot be certain this
is the explanation, but I have noticed all sorts of obscure things
that get "lost" in the translation to .doc format.
HOWEVER, for those folks who insist on using .doc files, challenge
them to produce the appropriate file themselves, on whatever version
of Word they use.... That's really the only way to ensure they get the
expected results from Word.
OR
They can print the letterhead paper using OpenOffice THEN run the
sheet back through the printer for the content of the letter using
another program. That way they can use the inkjet printer for color
letterhead and black laser printer for the text, for example, saving
lots of money on black ink (usually laser toner is cheaper). This trick
may not work well for multiple passes through a laser printer...
