Baffled by OpenOffice templates

I don't recall seeing that dialog; however I don't think I have used
any OpenOffice templates for items requiring addresses. I have used
mail merges (an often-frustrating experience in ooo) but you must be
describing a template with an embedded macro. Use one of the options
under Tools --> Macros to see what macros are installed, and you can
either edit it to suit your needs or delete the macro entirely.
