That thread gave me the idea to create pdfrecycle
<http://www.florian-diesch.de/software/pdfrecycle/>
With pdfrecycle you need a file like
--8<---------------cut here---------------start------------->8---
FILE rute.pdf
LAYOUT 1x1
MARK Title
PAGE 1-4
MARK Chapter Summery
PAGE 5-6
MARK Contents
PAGE 7-26
MARK Preface
PAGE 27,28
MARK Acknowledgments
PAGE 29,30
MARK Introduction
MARK1 What This Book Covers
MARK1 Read This Next...
MARK1 What Do I Need to Get Started?
PAGE 31
MARK1 More About This Book
MARK1 I Get Frustrated with U NIX Documentation That I Don't Understand
MARK1 Linux Professionals Institute (LPI) and RedHat Certified Engineer (RHCE) Requirements
PAGE 32
MARK1 Not RedHat: RedHat-like
MARK1 Updates and Errata
PAGE 33,34
--8<---------------cut here---------------end--------------->8---
Then run pdfrecycle to create a new PDF with that bookmarks
