Add this to your "Device" section in /etc/X11/xorg.conf:
# === Video Overlay for the Xv extension ===
Option "VideoOverlay" "on"
# Xvideo on external screen
Option "OverlayOnCRTC2"
# === OpenGL Overlay ===
# Note: When OpenGL Overlay is enabled, Video Overlay
# will be disabled automatically
Option "OpenGLOverlay" "off"
Note that Xv output will then be only and always to the external screen.
I have found no way to conveniently turn it on or off. Possibly
fglrx-control can set this up for you too, I can't remember.
Btw, I use the free "ati" driver and it shows Xv on both the internal
and external screen simultaneously without any setup work at all.
