How do I upgrade Xubuntu?

Did you follow the instructions in the Upgrade Notes [1]? It is
recommended to read the whole thing, but especially regarding
      * Installing a few meta-packages before upgrading
      * Having to run the 'apt-get dist-upgrade' at least twice.
