All of those values are very bad.  I would first look at your router.
Perhaps there's something wrong with your cabling?  Once your connection
to your router is clean, then start talking to the infrastructure people...

The reason why you're seeing all hops at 2-6% is that you are losing 2%
just to your router, so everything beyond that is going to see packet
loss, since everything is going through that router.  Since you're
already losing packets there, everything beyond that is suspect.

In a healthy network, you shouldn't be losing any packets until you hit
the commodity internet, if even then.
