Unable to access MiniSD Card

When trying to format a MiniSD card, inserted to a card reader I get this:

It belongs to a friend of mine whom used it for a while in a Nokia 6280
smartphone. She had a password set on it. after restoring the phone to
factory defaults, the card became useless - through the phone and any card
reader (under M$ too). When trying to format or change password in the
phone, it says the password is wrong, although it  is the right password for
sure (the date she got proposed by her boyfriend...)

Any thing I can do to help her?

Shame to waste it - it's a 2Gb card.

Is it possible to wipe the partition table to a fresh one?

She doesn't care for the data on it....
