I'm experiencing the exact same problem on Ubuntu Dapper : Can't get
Amavisd-new to add spam headers or modify the subject of spam mails to
include something like "***SPAM***".
SpamAssassin on its own works fine, but for a reason I can't understand
amavisd-new won't add headers/modify subjects.
My /etc/amavis/conf.d/50-user contains :

$sa_tag_level_deflt = -999; # add spam info headers if at, or above that
level
$sa_tag2_level_deflt = 5.0; # add 'spam detected' headers at that level
$sa_kill_level_deflt = 99; # triggers spam evasive actions
$sa_spam_subject_tag = '*****SPAM***** ';
$sa_quarantine_cutoff_level = 2000; #No quarantine under that level

If someone on this list has a working setup of
postfix/amavisd-new/clamav/spamassassin, I'd be curious to see some
config files to know what I missed.
