FWIW, I've encountered this (although on KnoppMyth, not on Ubuntu, and
with a zd1201 driver instead of orinoco_usb). There is a difference
between hotplugging and hotplugging at boot time -- aka. "coldplugging",
and it seems that some drivers at least can't deal with coldplugging.
Very annoying.

I'm currently thinking the difficulty is that 'hotplug' isn't running by
the time the module is loaded from /etc/modules, hence it can't get the
firmware file via that subsystem.  Haven't quite figured it out yet
though...
