My .forward file on a FreeBSD system where I use procmail is as follows:

"|IFS=' ' && exec /usr/local/bin/procmail -Yf- || exit 75 #zzalsaca"

where zzalsaca is my username. I think the last bit means deliver to
zzalsaca if the procmail process fails or something. It's a while since
I set it up and cannot remember the details right now.
