floppy gone missing

Hi:

My floppy drive recently disappeared from Ubuntu.  It's just a generic
ide floppy.  I reseated all of the cables, but it stayed gone.  Then I
booted Knoppix 3.7 to see if it was hardware or software, and the floppy
is present with Knoppix.  Does anyone have any ideas?

I've attached dmesg output and the contents of /proc/ide in case that
helps.
