Probably the driver for the physical devices on which the myth volume
resides is not loaded yet at the time that evms_activate runs.  This is
essentially https://bugzilla.ubuntu.com/show_bug.cgi?id=5204

You could work around it by adding the necessary modules to /etc/modules, or
changing the startup order as discussed in the bug.
