You can use dfsbuild (Debian From Scratch) or bootcd.  I did not have
success with bootcd but after about a week of reading and
experimenting I could install a second server with a CD build from a
first one with dfs.

There are articles about this subject on
http://www.debian-administration.org (covering at a few options for
building CD's).
