There seem to be several "niggling" problems with modem connections --
go to bugzilla.ubuntu.com and search for "modem" and you'll see
several.
Here are two that seem related to your issue:

I have been watching the modem bugs because I'm having lots of trouble
getting a modem-connected system back online after a Warty-to-Hoary
upgrade. Coincidentally the ISP I was using changed something as well,
so I'm having to work through changes on the system AND the ISP. (I
use a broadband connection, but the broken system is one I set up for
my Mother-in-law for her house. I keep hoping maybe she can borrow a
wireless connection from one of her neighbors, as she's not likely to
want to pay for her own broadband!)
It's a bit frustrating that the Modem Lights applet was taken away and
replaced with something inferior! I cannot even figure out how to
reinstall the Modem Lights applet on the panel. Time to open another
bug report....
