hoary menu editing: backwards not forwards?

The help system says that to change a menu entry you use the file
manager.  But there is no entry in the menu system corresponding to
'file manager' though there seem to be preferences for it. There is a
'file browser', but that doesn't seem to accept the URL that the help
system suggests I should use.

Perhaps I've overlooked something, or I'm being really stupid, but the
old system, where you just right clicked to add an item was lovely and
simple.
