mysql ERROR 1045 (28000): Access denied for user ...

I got the above error using mysql monitor as well as my php code and I am
unable to find the error.  I have searched a lot of forums and there is a
lot of verbiage, but no solution.

I would appreciate any ideas anyone might have.
