[ubuntu-uk] Upgrading Ubuntu Hardy Heron 8.04 server to latest version? And when has an Ubuntu server "crashed"?

Install update-manager-core if it is not already installed:

 sudo apt-get install update-manager-core

Launch the upgrade tool: sudo do-release-upgrade

Follow the on-screen instructions

If its pinging then something is there, Anything in the logs?
