i have applied the fix, however the issue still happened :-(

ii  kpartx                           0.4.9-3ubuntu5+lp1032250dbg1             create device mappings for partitions
ii  multipath-tools                  0.4.9-3ubuntu5+lp1032250dbg1             maintain multipath block device access
