LP: 1422113 - "Missing complete icon" fixed.

Fix:  updated ubiquity.ui file to reference correct icon files instead of legacy files, which no longer work.  Fixed not only the "information" dialog icon reference but "warning" dialog icon references.

Rationale:  Incorrect file references caused icons to be missing from Ubiquity front end dialog boxes.  This needed to be corrected

Testing:  Build the source on vivid.  Boot up an Ubuntu-Gnome vivid LiveCD and select "Try Ubuntu".  Use scp to copy all the .deb files from your build to the LiveCD filesystem.  Install .debs using "dpkg -i".  For me, dependencies could not be resolved by dpkg so ran "sudo apt-get -f install" to fix dependencies.  Run ubiquity until completion and you will now see the "light bulb" icon instead of the missing icon.
