First, FF is complaining about a corrupt extensions.sqlite file, so try
this:
http://kb.mozillazine.org/Unable_to_install_themes_or_extensions#Corrupt_extension_files

If that doesn't help, try running firefox in safe mode and let us know if it still crashes in safe mode:
firefox -safe-mode
