This happens in Edgy and Feisty with whatever version  of FF is the
latest in their repos.   I think it also happened with Dapper.

set up

  * sudo aticonfig --dtop=horizontal --enable-monitors=tmds1,crt1
  * have laptop disconnected from dock or any external monitors
  * sudo /etc/init.d/gdm restart
  * login
  * invoke firefox (nothing happens)
  * invoke it again (nothing happens)
  * killall firefox-bin
  * xrandr -s 1
  * invoke firefox (FF window comes up)
