debian-cd OEM image error

I'm trying to create an OEM Ubuntu installation CD. my goal is to
create an Ubuntu installation  CD that only installs my pereffered
list of packages on a specific machine. For that Im using debian-cd
port at: http://bazaar.launchpad.net/~ubuntu-cdimage/debian-cd/ubuntu

I've managed to create OEM mode image, and its integrity check passes.
but in "Load installer components from CD" stage it gives the
following error message:
There was a problem reading data from CD-ROM. Please make sure it is
in the drive. If retrying does not work, you should check the
integrity of CD-ROM.

Is there a way to know what the problem is? is there a file missing?
is it a dependency problem? or is it a driver problem?
please help.
