Proxy UI in Ubiquity

Hi Jonathan,

As we discussed on IRC at the end of last week, I've implemented HTTP
proxy configuration in Ubiquity's Advanced dialog for the GTK frontend,
but my Designer skills are a little bit too rusty to manage the Qt
version. Could you (have somebody) take care of this, please?

For reference, the attached patch is the relevant part of r2445 that
implements the GTK side. (I stripped out the changelog and translations
parts of the patch.) As you can see, there isn't much to it - basically
just an extra text entry box for the proxy host name and a spin button
for the port number.

This is for the networkless-installation-fixes specification.
