Changes to add-applications

Hi guys,

I've attached a patch which rearranges and simplifies add-applications.
The aim was to make the whole section more concise, and to keep all of
the conceptual information separate from the instructions.

Please let me know what you think. I'd ideally like to commit this in
the next few days.
