Understanding documentation fixes

A friend of mine has just installed Ubuntu and found this bug in the
documentation while trying to trouble shoot:

Open Device Manager (System → Administration → Device Manager).
It should read:
This should be: System -> Preferences -> Hardware Information.
After much confusion on the IRC Channel it was discovered that this fix
has already been made:

I was *completely* impressed that my engineer friend bothered to read
the documentation. I was slightly embarrassed that there was an error in
it and said I'd look into it. Now I'm confused as to why there has been
a fix made, but it hasn't been applied. This is the Interweb! It should
be easy to publish fixes and correct the online documentation.
What is the policy for fixing stable release documentation? Surely we
don't leave it wrong in perpetuity...
