Documentation Survey

Hi guys,

Thanks for all of the replies. I think it would be best to go with a
hosted survey service with some built-in data analysis tools. We also
need to publicise the survey once it's published so we get a good sample
of people.

I've attached a first draft. Comments/changes are very welcome.
