some new pdfs

Hi there,

I've continued my permanent wrestling match with fop, and have changed
some things in the pdfs. Here are the latest examples, built with a more
recent version of fop, which has of course solved some problems and
introduced others.

Please give your feedback!

Current known problems include:
 * random disappearance of some footnotes
 * left hand page numbers are not in line with the margin
 * some weird rendering of the page numbers after cross references

Hopefully the various mailing lists that I spam with pdf problems can
help.

Brian has also produced some shiny new covers so things are going quite
well.
