strange upgrade bugs with "cannot access archive: No such file or directory"

hi folks,
While going through my bugmail I've noticed a number of bugs with the
same pattern that are assigned to various packages by bug triagers. All
Terminal logs look like this:
dpkg: error processing /var/cache/apt/archives/libmp3lame0_3.98.2+debian-0ubuntu1_amd64.deb (--unpack):
cannot access archive: No such file or directory
With googling it seems that the following bugs follow that pattern.

I'm a big puzzled how these failures can happen. Did the file somehow
got deleted during the install process? I cannot imagine that some user
is calling an 'apt-get clean' deliberatly simultanous to running an
dist-upgrade, so I suspect something else is doing this.
Moreover, perhaps bugsquad has some infrastructure to batch-process this
kind of bugs?
