I've had occasional problems booting a machine from a USB disk, I recommend
trying to boot from a CD (that you check for errors) to see if that works,
since that may have an updated version of network-manager that doesn't have
the issues anymore.

You'll want to take a look at the NetworkManager Mailing List, which is
found at http://mail.gnome.org/mailman/listinfo/networkmanager-list,
presumably they will be better equipped to let you know what's going on.

If you're comfortable building your own source, I've heard that the new
0.7.1 version of Network Manager has better support for wireless 3G cards.
After running: sudo apt-get build-dep network-manager, you can find the most
recent release at:
http://ftp.gnome.org/pub/GNOME/sources/NetworkManager/0.7/NetworkManager-0.7.1.tar.gz
