Changing the launchpad team structure

Hey all,

I finally got around to do something that I had wanted to do for a
long time. The Canadian Team is now open subscription and I have
created a new team - ubuntu-ca-members for those who are ubuntu
members from Canada. If you are an Ubuntu Member, feel free to apply
to join the new -members group.
