http://wiki.ubuntu.com/DebuggingSoundProblems which contains a section
on "Reporting Sound Bugs".  This is actually a part of a series of
debugging pages, https://wiki.ubuntu.com/DebuggingProcedures, which
contain information for reporting bugs on a variety of packages.  Those
may be handy in the future.
